Credit Reports

The three Credit Bureaus now require additional documentation to access consumer credit information. Depending on which Bureau used, please download and read the following required documents and sign the Signature page(s). Fax signed page(s) to (813) 277-3660.

  MAF Addendum A and Sample Letters of Intent

• All customers must sign and return page 4 of Addendum A and a signed Letter of Intent for the use of any credit information.
   

 Trans Union

• Customers wanting Trans Union Reports must Read Trans Union Appendix C-1 (no signature required)
   
• Customers wanting Trans Union Credit Scoring must ReaD AND RETURN Trans Union Appendix C-2

 Equifax

• Customers wanting Equifax Reports must sign and return Equifax (pages 6,7,9 & 11)

 Experian

• Customers wanting Experian Reports must Read Experian’s Appendix B (no signature required)
