Credit Reports
The three Credit Bureaus now require additional documentation to access consumer credit information. Depending on which Bureau used, PLEASE DOWNLOAD AND READ TEH FOLLOWING REQUIRED DOCUMENTS AND SIGN THE SIGNATURE PAGE(S). Fax signed page(s) to (813) 277-3660.
MAF Addendum A and Sample Letters of Intent
All customers must SIGN AND RETURN PAGE 4 of Addendum A and a SIGNED LETTER OF INTENT for the use of any credit information.
Trans Union
Customers wanting Trans Union Reports must READ Trans Union Appendix C-1 (no signature required)
Customers wanting Trans Union Credit Scoring must READ AND RETURN Trans Union Appendix C-2

Equifax
Customers wanting Equifax Reports must SIGN AND RETURN Equifax (pages 6,7,9 & 11)
Experian
Customers wanting Experian Reports must READ Experian’s Appendix B (no signature required) and Complete and Return the Experian Credit Scoring Services Agreement
