If you want to join Merchants Association of Florida, Inc. to access credit, criminal, mortgage, collections and other services, a completed membership application is required. Credit Reports
The three Credit Bureaus now require additional documentation to access consumer credit information. Depending on which Bureau used, please download, sign and return the Signature page(s) with your application.
MAF Addendum A
- All customers must sign and return the last page of Addendum A if you will be using any credit reporting service.
Trans Union
Equifax
- Customers wanting Equifax must sign and return Equifax (pages 6,7,9&11)
Experian
- Customers wanting Experian reports must read Experian’s Appendix B (no signature required)
Motor Vehicle Reports
This requires a special agreement to ensure compliance with the many different state requirements. Please download, complete, sign and return the addendum to the address above.
