Credit Reports
TThe three Credit Bureaus now require additional documentation to access consumer credit information. Depending on which Bureau used, please download, sign and return the Signature page(s) with your application.
Letter of Intent
MAF Addendum A - All customers must sign and return the last page of Addendum A if you will be using any credit reporting service.
Trans Union Appendix C-1 - Customers wanting Trans Union Reports must read Appendix C-1 (no signature required).
Trans Union Appendix C-3 - Customers wanting Trans Union Credit Scoring must read and return Trans Union Appendix C-2 (enter type of business on page 1).
Equifax Appendix - Customers wanting Equifax must sign and return the Equifax Appendix (pages 6,7,9, and 11).
Motor Vehicle Reports
MVR Addendum - This requires a special agreement to ensure compliance with the many different state requirements. Please download, complete, sign and return the addendum to the address above.
